following three items to the address on the official request form.

1) The original UPC code from the carton of any Centrum® OR
Centrum® Silver®. For 30 count and for 150 count or larger, only
the original cash register receipt is required.

2) The original cash register receipt dated between 7/12/09 &
8/15/09 with the price circled.

3) The completed official request form
